Patient Privacy
Eberbach Plastic Surgery is required by law to keep your medical records private and secure. You have many legal rights under the Health Insurance Portability and Accountability Act.
What is HIPAA? HIPAA is the Health Insurance Portability and Accountability Act. Article II of the law regulates the use of individual's protected health information. We keep medical records of our patients and the disclosure of the information in these records is strictly enforced through HIPPA. We are required through HIPAA to inform of your rights to privacy and what our obligations are under the HIPAA act.
Your rights
You have the right to access and copy medical information from your files. You may submit a written request to our office. We are required to respond within 30 days if the record is readily accessible and within 60 days if it is not readily accessible.
- You have the right to make corrections to medical information in your chart where entries may be inaccurate or incomplete in your file. You can do this with a written request. Once again, we are required to respond within 60 days.
- You have the right to request restrictions as to the amount of medical information we disclose within certain limitations. For a full explanation of the limitations, please contact our office.
- You have the right to request that all communications concerning your records remain confidential. We at Eberbach Plastic Surgery guarantee that all communications in our office are confidential.
- You have the right to receive an explanation of any disclosures made from your file over the last six years. You can obtain this with a written request directed to our office. We must again respond within 60 days.
